Privacy Collection Notice

Self-Managed Support Pty Ltd trading as HomeMade Support (ABN 88 638 372 960) (‘HomeMade’) delivers package management services as a registered aged care provider in accordance with the Aged Care Act 2024 (Cth), under which HomeMade is authorised to collect personal information.

Why is HomeMade collecting Personal Information about you?

HomeMade collects Personal Information about you so that it can provide its technology enhanced aged care solution to its Customers and Users. This includes to:

  1. Provide and facilitate Care Services to Customers;
  2. Deliver and improve the digital Platform; and
  3. Deliver safe and compliant solutions for Care Services at home.

 

If you do not provide us with personal information, we may not be able to create your account on our Platform, approve you as a User of our Platform or respond to your queries.

What is HomeMade collecting about you?

HomeMade will collect your personal and sensitive information from you. For Customers, Service Providers, Representatives, Coordinators, Supporters and Substitute Decision Makers, this includes, but is not limited to:

  • Your name, date of birth, other information by which we can identify you or verify your identity (such as driver’s licence), contact details such as email, mailing address and phone number, gender, cultural background, interests and any other information you provide us.
  • If you are a Service Provider, HomeMade will also collect your ABN, information about your employees or personnel that provide services to your customers, work-related information, information about your employer or an organisation that you represent, criminal history, the results of any applicable screening checks, work experience, income, professional license numbers, education, professional and personal references, professional skills and abilities, lifestyle information, schedule availability, and bank information to facilitate direct deposit of earnings.
  • If you are a Customer, HomeMade will also collect your family or emergency contacts, care services needed, support plan, health conditions and information, package information (including referral code and aged care ID), household information and credit card or bank information to facilitate payment processing.
  • If you are a Representatives, Coordinators, Supporters or Substitute Decision-makers, HomeMade will also collect your relationship with the Customer and Substitute Decision-Making documentation (such as an Enduring Power of Attorney).
  • Any personal information you provide to us/authorise us to collect, as part of your interaction with HomeMade.

For visitors to our Site, HomeMade will collect personal information that you provide to us, including through submitting forms, feedback forms, messages, saving files to your account, AI assistant tool or other website tools.

Who does HomeMade collect personal information from?

HomeMade will typically collect your personal information from you but may also collect it from third parties such as if someone provides the information on your behalf, such as a Representative, Coordinator or Substitute Decision Maker, and from Service Providers, government departments or our third party suppliers and technology.

Who does HomeMade share personal information with?

HomeMade protects personal information as a priority, including when HomeMade discloses information to another party.

We may disclose your personal information to:

  • Other Users of the Platform, such as Service Providers or Customers (as applicable);
  • Coordinators or Community Partners under our Partner Coordination model as applicable;
  • General practitioners and other healthcare providers;
  • Authorised Representatives, Supporters or other contacts on your account;
  • Our consultants, advisors and other third parties who may work with us to improve or develop our Site and Platform, operate or develop our business, conduct surveys or seek feedback from you, conduct market and other research, or facilitate or collaborate with us in relation to promotions;
  • Our Affiliates/related entities, including Mable for certain product offerings or otherwise with your consent;
  • Third parties we engage to deliver our services such as contractors and service providers; and
  • As legally required or in other legally permissible situations.


We may share your personal information with trusted external service providers located outside of Australia. Before doing so, HomeMade takes steps to ensure that Personal Information is protected to a similar or higher level as is required in Australia or requires external parties to operate within HomeMade’s Australia-based servers. We may process data through servers that are located outside of Australia, such as in the European Union, but data is not stored overseas other than temporarily at rest and those entities cannot use or disclose any Personal Information.
