HomeMade TeleNurse

A simple way to stay on top of your health

Get easy access to an experienced Registered Nurse who understands your care and Support at Home funding.

Talk to a nurse about what’s on your mind

You don’t need to wait until something feels serious. Regular nursing check-ins can help identify small concerns early and give you confidence about what to do next.

A HomeMade TeleNurse can help with:

  • Falls prevention
  • Continence support
  • Medication and pain management reviews
  • Advice about nutrition and hydration
  • Mental wellbeing
  • Other health concerns

How it works

Book a time

Choose a time that suits you.

Speak with a nurse

Have your appointment by phone or video. There’s no referral or waiting room required.

Get clear advice

Your nurse will listen, answer your questions and talk through any practical next steps or further support that may help.

Please note: to qualify for this service you, or your loved one, must have nursing approval in your My Aged Care Notice of Decision. You can view your Notice of Decision on your My Aged Care Online Account.

What does it cost?

HomeMade TeleNurse is paid from your available Support at Home budget.

Pay for the time you need

Appointments are charged in 15-min blocks. Most are around 45-min.

$47 per 15-mins

As a clinical support, there’s no customer contribution to pay.

Not sure if TeleNurse is right for you?

You don’t need to work out whether your concern is “important enough” first. If something is on your mind, talk it through with a nurse.

You can book a call if:

  • something about your health has changed
  • you have a question about your medication or pain management
  • you’ve had a fall or are worried about your balance
  • You would like advice on supporting your continence
  • you want advice on nutrition or hydration
  • you’re concerned about your mental wellbeing
  • you’re not sure whether a health concern needs further attention
  • you’d simply like a regular nursing check-in

Make the most of your Support at Home funding

Only a limited amount of unused Support at Home funding can be carried into the next quarter.

Any remaining funding above this limit is returned to Services Australia, so it’s worth using the funding available where it can genuinely support your health and care needs.

If you have Nursing Care included in your Care Plan and available funding, HomeMade TeleNurse may be one way to put that funding towards your health.
