For information on funding levels and accessing AT-HM, refer to our Understanding Assistive Technology and Home Modifications funding guide.
What is assistive technology?
Assistive technology includes mobility aids, personal alarms, and bathroom equipment to help you live independently.
For a full list of what can be covered with AT-HM funding, refer to the government’s AT-HM list.
What do I do once I have the funding?
Once you have been assigned funding, the next step is to contact your HomeMade Support partner to update your care plan and budget.
We will go through everything with you, including:
- A pre-purchase review and approval process
- Equipment delivery and set-up costs
- HomeMade’s administration fees
- Prescription costs and referrals, if relevant.
Please note: Your care plan and budget must be updated before any items or supports are purchased.
How do I organise purchasing equipment?
You will need to organise the purchase of the equipment through HomeMade, including:
- Ensuring your chosen supplier is onboarded with HomeMade
- Obtaining quotes from suppliers
- Providing proof of prescription, if relevant.
In some instances, you may need to obtain a prescription before purchasing the equipment. If you are unsure if this is the case, contact us so we can help guide you through the process.
Once we have all the relevant documents, we’ll contact you and the supplier to confirm the equipment is ready for purchase. The supplier can then submit an invoice, so we can process the payment.
What if I transitioned from a Home Care Package?
If you have moved to Support at Home from an existing Home Care Package (HCP), any remaining Commonwealth unspent HCP funds must be used before accessing any Assistive Technology and Home Modifications (AT-HM) funding.
You can use your unspent HCP funding to pay for assistive technology items. Your Support Partner can confirm how this applies to your individual funding. Please book a care plan review to discuss your funding options by getting in touch with us.
