What are home modifications?
Home modifications include changes to your home to make it safer and more accessible, such as grab rails, ramps or bathroom modifications.

What do I do once I have the funding?
Once you have been assigned funding, the next step is to contact your HomeMade care partner to update your care plan and budget.
We will go through everything with you, including:
- Pre-works review and approval process, including obtaining quotes
- Prescription costs and referrals
- Total project costs
HomeMade administration fees
Please note: Your care plan and budget must be updated before any items or supports are purchased.
Do I need a prescription for home modifications?
All home modifications require a prescription from an occupational therapist. They will:
- Assess your home and needs
- Recommend specific modifications
- Provide a prescription outlining the required work.
How do I organise purchasing equipment?
First, you must provide documentation regarding all costs to HomeMade.
You will need to organise the purchase of the equipment through HomeMade, including ensuring your chosen supplier is onboarded with HomeMade and, if relevant, providing proof of prescription.
Please note: Before any work takes place, it must be approved by HomeMade and your referring service provider.
What if I rent or live in government-provided public housing?
If you rent the home you live in, you will need to contact your landlord to provide written consent for the home modifications.
If you live in government-provided public housing, you may not be eligible for home modifications. This is because, in most cases, home modifications are provided through state or territory housing schemes.
Do I need to do any follow-up once the work is complete?
Follow-up may be required to ensure they are safe and effective.
This may include:
- An occupational therapist review
- Education or training on safe use
- Adjustments.
What if I transitioned from a Home Care Package?
If you moved to Support at Home from a previous Home Care Package (HCP), any remaining funds must be used before accessing AT-HM funding.
Your support partner can confirm how this applies to your specific funding.
